Medication and Health Forms
Student Medication Policy
NUSD Health Services staff advocate for healthy school environments and student success. In accordance with state guidelines, a current medication authorization form must be on file for any student requiring medication during school hours. These forms ensure that our nursing staff can provide accurate, safe, and physician-directed care.
All medications—whether prescription or over-the-counter—require a signed authorization form. This includes all delivery methods, such as liquids, pills, eye drops, nasal sprays, and topical treatments.
Whether your child requires daily medication, an emergency medication, or medication as needed, California Education Code requires a completed medication form each school year signed by both a parent and a healthcare provider. You can download these forms here and return them to the school health office at your convenience.
Please reach out to our School Nurse with any questions or concerns.
