Benefit Selection Sheets

If you are making changes to your health benefits, you must complete, sign, and submit a Benefit Selection Sheet. These sheets are available below. Additionally, if you are changing providers or adding dependents, you will need to fill out the provider enrollment forms and submit dependent verification forms.

All documents must be submitted to the Payroll and Benefits Department by October 11. You can send them via district mail, regular mail, drop them off in person at the front desk, or email them to payrollbenefits@natomasunified.org.
