Certificates of Insurance

Complete the Certificate of Insurance Request Form. After completion please forward the Request Form along with the agency/vendor contract (signed by a site administrator) to the Business Services office.  (The most common agency/vendor examples include Westminster Woods Camp & Conference Center, Alliance Redwoods Conference Grounds, Inc. and Grizzly Creek Ranch.)

Special Events Insurance

Some activities are not covered by SIA (NUSD insurance provider) and classified as “High Risk”; such as Inflatables (including bounce houses), Water Slides, Rock Climbing Wall, Trampoline, Dunk Tank, Sumo Suit Wrestling, Animal Rides, Velcro Jumping, Zip Line, Obstacle Course and more.

If your function involves a High Risk activity and is a site sponsored event (not a PTA or PTSA event), please complete the Special Events Application and forward it to Business Services. SIA requires that all vendors – participating at the event – provide a certificate of insurance naming Natomas Unified School District as additionally insured with the additional insured endorsement.  These certificates (one for every vendor) must be attached to your request.  Please allow for a two week processing time.

Please note that the “High Risk” activities listed above (and many more) are generally prohibited under PTA insurance policies.  In some instances, the PTA may purchase additional coverage from their insurance provider; however, the Business Services office must be notified 2-3 weeks in advance at (916) 567-5457 when any “High Risk” activities are being planned for use on District Property.
